A breast lift or Mastopexy is an aesthetic (cosmetic) surgical procedure aimed at lifting and reshaping sagging breasts to a rejuvenated shape and contour.
Breast lift procedures are indicated if the breasts are sagging or drooping and have lost their shape, particularly fullness of the upper poles. You may have noticed this drooping or "ptosis" with time or following breast feeding, loss of weight or with ageing. Mastopexy lifts and resuspends the breasts and helps improve your overall appearance and self-esteem.
It is not uncommon to seek a breast enhancement and be advised at the time of consultation that a skin tightening or lifting as well as an implant will be needed to achieve the desirable result.

The surgery is generally performed on an outpatient basis. The procedure can be done with local anaesthetic and sedation or under general anaesthesia. The operation usually takes about 2 hours. There is no need for taping after the procedure and dissolving sutures are used so none need to be removed afterwards. Generally you will be able to go home 2 hours after surgery.
Some discomfort may arise from the stretching of the breast tissues, but it largely resolves within two to three days and is well-controlled with simple oral medication such as Panadeine. Showering is allowable 48 hours postoperatively.
Patients generally return to work within one week, but should avoid exertional activities over the 6 to 8 weeks required for bruising and swelling to resolve.
What about Complications?
Breast augmentation is an open surgical procedure that is associated with a low incidence of complications. At your consultation these will be discussed with you and you will be able to ask any questions you have about them.
Post-operative complications include blood accumulation or infection adjacent to the implant. Both problems occur infrequently, but can necessitate a second operation or temporary removal of the implant. Because implants are a medical device, they can fail at any point after implantation. This is uncommon, but may require implant replacement surgery.
Some patients develop a firm layer of scar tissue around their implants which can make the breast hard to the touch or painful. Called "capsular contracture," this can cause the breast contour to be asymmetrical and may require secondary procedures for improvement. Nipple sensation can be increased or decreased by the procedure though most women will experience some decrease in sensation around the nipple and outer breast which will be more noticeable in the few months following surgery but will improve with time. Most patients are able to breast-feed following augmentation.
The scar following breast augmentation is 6 cm long and sits under the breast in its crease. The scar will be reddened and more noticeable for 6 months or so but can be expected to fade dramatically over 12 to 18 months.
Breast Screening
Breast cause very little interference with the ability of mammography to detect early breast cancers. This is an important consideration that patients should discuss with a physician, particularly patients with a first-degree relative who has had breast cancer. All augmented patients need to inform their radiologists of their implants so the screening technique can be favourably modified.
Despite these limitations, approximately two million women are estimated to have undergone breast enlargement surgery. The overwhelming majority are satisfied with the results.
How important are psychological factors in relation to this procedure?
Emotional stability is the primary factor to be considered before any aesthetic surgery is performed. A "new body" does not guarantee a new life or solution to personal problems. It is mostly the mental attitude of the individual that determines a successful outcome. Breast augmentation can improve your appearance and renew your self-confidence. It is a procedure you do for yourself, not for anybody else.

How is mastopexy performed?
There are currently several techniques for mastopexy. They all aim to lift, reshape and recontour the breasts, and reposition the nipples. At consultation your physical condition and requirements will be considered, and the technique that would be best for you will be determined.
If the breast sagging is minimal and the breasts are small, ptosis can be corrected with augmentation mammaplasty. This method causes fewer scars on the breast than mastopexy.
During mastopexy the nipple and areola are repositioned to a higher position and excess skin is removed from beneath the breast. Closing the remaining skin lifts the breast mound to a higher position and recontours the breast to give it a more projected and youthful appearance.
The commonest way Dr Drielsma offers breast mastopexy is using the Lejour vertical mammaplasty technique. This method reduces scarring associated with mastopexy by eliminating the scar under the breast. The resulting scar pattern runs around the nipple and vertically down the middle of the lower breast (lollypop pattern). In some cases, the periareolar technique can be used resulting in a scar only around the nipple. The suitability for these techniques will be clarified at the time of consultation. Scars fade and soften to be almost unnoticeable over a year to 18 months.
If there is inadequate breast tissue to fill the skin and achieve the desired size, a breast implant may be placed beneath the breast at the time of mastopexy, achieving both the desired size, shape and projection.
An overnight stay is usually necessary and you may need around 1 to 2 weeks off work, back to normal excercise etc in about 6 weeks. Pain with this type of surgery is surprisingly little and can usually be controlled with oral pain medication. Stitches around the nipple are removed 1-2 weeks after surgery.
The operation may be covered by private health insurance in terms of Hospital costs and Surgical costs may be partially recoverable from medicare and your private health fund.
Are there any side effects of this procedure?
The scars are situated around the areola of the nipple and under the breasts and usually fade over 12-18 months. Thus, although scars will be present, most patients believe they are worth it. Bleeding and infection are uncommon complications. There may be an alteration in sensation of the nipple, but this often improves with time. Current techniques of breast mastopexy may preserve the ability to breast feed but you should discuss this with Dr Drielsma. There is normally a slight difference in size between the breasts and this may persist following your procedure.
